• ID card with name, and group number will be issued to each member
• 5-10 employees and person @ rate $30 per person
• 10 employees or individuals and their significant other = $500 per month
• Between 10-15 employees or individuals and their significant other = $ 750 per month
• Between 16-30 employees or individuals and their significant other = $ 1000 per month
• Over 30 employees or individuals add $ 30 per person
• Renewal month to month, auto renewal option available
• No contract needed
• No insurance needed
• No controlled medication will be prescribed.
